The Standard NHS CHC Process
The Standard NHS CHC process is to be applied only once the fast track option has been considered. If fast track is deemed not to be applicable then a two phase Standard NHS CHC is applied. Phase 1 is the inital filtering stage termed the NHS CHC Checklist where trained healthcare professionals discuss with you your ongoing healthcare needs using as a base the standard nationwide checklist. Phase 2, upon successful completion of the NHS CHC checklist then you will move forward to the NHS CHC final decision using as the base the standard decision making tool (DST).
Fast track
Continuing Healthcare Fast-track is completely separate from standard Continuing Healthcare which goes through a two-phase process. For Fast-track the only requirement for eligibility for fast-track funding is;
- That the individual has a rapidly deteriorating condition.
- The patient may be entering a terminal phase.
Any clinician can complete a fast-track assessment on behalf of the patient and submit the same to the Clinical Commissioning Group.
NHS Funding is provided in over 90% of fast-track cases and now fast-track represents 70% of all referred CHC cases. Standard Continuing Healthcare funding is now being approved in only 25% of cases.
